Supporting Documents for IV (DS-260)

Here is what I'm submitting for my IV supporting documents package (my wife is the beneficiary/applicant):

Cover Sheet with bar code

Cover Letter listing contents

2 passport photographs of my wife

Copy of the biographical data page of my wife's passport

Certified copy of my wife's birth certificate (from NSO)

Certified copy of marriage certificate for my wife and I (from NSO)

Original NBI Clearance for my wife (green copy)

A photocopy of the birth and marriage certificate listed above

Is that everything I need?

Note: My wife does not have any prior marriages; she has never been outside of the Philippines; and we have a daughter.

What you have is correct. You don't need a photocopy of the photographs, barcode, or cover letter. I would include a photocopy of the police clearance report just in case. The passport bio copy IS a photocopy and so you don't need to submit 2.

Include the photocopy of the police clearance report and I think you're good to go! :)
