NVC doesn't send u anything but the link to the website to get everything yourself.
That's why most on VJ have everything prepared in advance.

As DHwheels said you have to go download it. They wont send any forms to you.

October 26, 2012: Picked up visa from DHL (relieved - delay caused by Sandy)
December 15, 2012: POE Atlanta.
