Sample of Parental Consent Letter

I am 21 and I have heard that I need to bring a parent's consent for the CFO seminar. The problem is I don't know where I can find form or whatever is it needed to get this Parental consent. Could anybody be kind enough to show me their format or sample of parent's consent.

What country are you from? This was not a requirement for us in Brazil, so it must be a country specific requirement...

Here is a sample of parental consent, I don't know if it needs to be notarized by a lawyer

but I prefer to have it notarize, won't cost too much if you prepare your own letter.

PARENTAL CONSENT

solemnly swear that I am the [ ]father [ ]mother [ ]lawful guardian (1) of

___________________________________________________________

who is _______ years old on this date, and I do expressly give my consent and authority

to the marriage of my daughter__________________________________.

Signed BEFORE ME, Notary Public, this ______day of July, 2012,

I have heard of at least one case where the CFO would not accept the written affidavit and required the parent to appear in person. Perhaps the affidavit was not executed properly, or the CFO had some other reason to reject it.

We had ours notarized at the city hall and that was enough. Mom went with and they didn't want to see her, she had to sit outside..

I am 21 as well and had my CFO last week (august 1) they got my 2 letter of consent (one is parental consent that i made for my parents and have it notarized and one is http://www.quezoncity.gov.ph/images/stories/downloadables/forms/parentadvice.pdf)
