Sample of Parental Consent Letter

I am 21 and I have heard that I need to bring a parent's consent for the CFO seminar. The problem is I don't know where I can find form or whatever is it needed to get this Parental consent. Could anybody be kind enough to show me their format or sample of parent's consent.

What country are you from? This was not a requirement for us in Brazil, so it must be a country specific requirement...

Philippines issue only.

Here is a sample of parental consent, I don't know if it needs to be notarized by a lawyer

but I prefer to have it notarize, won't cost too much if you prepare your own letter.

PARENTAL CONSENT

I,______________________________________________________________ do

solemnly swear that I am the [ ]father [ ]mother [ ]lawful guardian (1) of

___________________________________________________________

who is _______ years old on this date, and I do expressly give my consent and authority

to the marriage of my daughter__________________________________.

Subscribed and sworn to before me on the ____________ day of August, 2012

__________________________

Parent/Guardian Signature

Signed BEFORE ME, Notary Public, this ______day of July, 2012,

In the City of Iloilo, Philippines, personally Appeared _________________with

his/her Community Tax Certificate No.__________, Issued in __________ City,

on _________ 2012, known to me to be the same person who executed the

foregoing instrument, and they acknowledged To me that the same is their

free and voluntary act and deed.

I am 21 as well and had my CFO last week (august 1) they got my 2 letter of consent (one is parental consent that i made for my parents and have it notarized and one is http://www.quezoncity.gov.ph/images/stories/downloadables/forms/parentadvice.pdf)

Actually meron din sila sa city hall nyan pero kasi meron nkong dala kya d nko ng hingi copy, pina notarized ko nlng sya..
