Hello everybody! I'm going to use your help again. I am a K-1 visa holder, i married my husband last Saturday and we are working on the AOS package.

I'm just confused about the I-693 form. I got all the vaccinations i needed before my visa interview and i know that i'm not required to have another medical examination since last one was very recent, but do i still need to go to a civil surgeon to complete the form?

Thank you in advance!

hi aleplay,

pls check your vaccination if all the necessary vaccines needed is complete then stil u need a civil surgeon to transcribe your vac history,,the civil surgeon will supply u a form 1-693 and you will have to submit the form ds-2053 fr the physician in ur country when u file AOS,thus, make a copy of the vaccination u had just in case they lost it or something, and then bring it to the civil surgeon also u can find the sc online at uscis.go

u need the ff when u go for cs:

passport

ds 2053

marriage certificate

and yourself,

payment...

take note that there are some cs that dont know about k-1 u have to explain to them that u dont have to do all over again the medical just the vac that needed it will take 2 to 3 days to get it done,PLS MAKE A CALL FIRST BEFORE MAKE A VISIT TO THE CS.

hope it will help and enlighten your mind :innocent:

If all your vaccinations are complete, then you do not need to see a civil surgeon. Is your DS-3025 from the medical marked complete or incomplete? Do you have all the vaccinations required for AOS, such as the MMR, tDap, and varicella(if you had chicken pox, you don't need this one)? If you are missing any, you need to see a CS and have your vaccine records transcribed. You only need Part 1, Part 2-Vaccinations Section ONLY, and Part 5 filled out. Sometimes it takes a bit of calling around to find one that will just do the vaccinations.

Follow this info about your DS-3025 to know if you need to see a CS or not: http://www.visajourney.com/forums/topic/315533-i-693-or-ds-3025/page__p__4732435#entry4732435

That post and thread has a lot of good info.

If you do end up needing additional vaccinations, you can usually get them cheaper at a county or gov't health clinic. Take a copy from the health clinic to the civil surgeon and they will transcribe them.

Yes. Since Juarez does not give vaccinations to those applying for a K-1, you will need to find a civil surgeon and have them fill out an I-693 for you. Call around until you find one that will do the vaccination transcriptions only. You do not need a full medical again, and many civil surgeons will try and insist you do and charge you for it. You can print out the instructions for the I-693, paying attention to Section 3 FAQ #2, from the USCIS website, and take that with you to show that you are adjusting from a K-1 and do not need a medical. Sometimes you have to point it out to them in writing. You only need to fill out Part 1, Part 2-Vaccinations Sections ONLY, and Part 5. Bring any vaccination records with you, and they can transcribe those over onto the I-693 for you. The civil surgeon can tell you which vaccines you need. You can get the shots you need somewhere cheaper, like the public health clinic, or get any you need directly from the civil surgeon while there.

This is the list from the USCIS: http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=3384cc5222ff5210VgnVCM100000082ca60aRCRD&vgnextchannel=6abe6d26d17df110VgnVCM1000004718190aRCRD

I have been looking and looking for vaccine info and this seems to be the most up to date. Is this still the correct sheet? Or is it best to call a CS in the US and ask them what is needed?

No you don't. As long as all your vaccinations are up to date. And your K-1 medical exam is still with one year. You don't have too see another civil surgeon. We didn't and we got approved.

Your profile still says "AOS (pending)"

No blood test, TB test or "review of x-ray" was required. If your shots were incomplete you have a vaccination transcription done. No parts of the medical though.
