Changing last name on my Philippine Passport

Sorry guys, I didn't know exactly which forum to post this on so I just chose "Off Topic" :)

I want to get my Philippine Passport amended and change my last name on it to my married last name... so my question is, do the Philippine Passports still show your "Maiden Name" in the biographic page once you get it amended to change your maiden surname to your married surname? I tried to google some Philippine Passport imagess to see some examples but they all just show Last, First, and Middle name.... For my middle name, will they just put my maiden name there??

For all of my immigration documents, I've already changed my surname (to my husband's) and just totally dropped my maiden name and kept my middle name. So now, I go by my (first name) (middle name) (married last name)...

Do you guys just suggest that I tell the Philippine Consulate that I DON'T want my maiden name to show... or???

You will get more answers in the Philippines Regional forum. To get you started, you can renew your passport, get it amended, or get an affidavit of one and the same. All that can be done at the Philippine Consulate for the region in which you live. You also have to do a report of marriage. The name as it appears will be decided by the consulate, according to Filipino tradition. If your US ID differs in form, you may need the affidavit of one and the same, especially when boarding in the Philippines and going through Philippines customs, just to be certain of no hassles.

My wife just renewed her passport with her married name hyphenated maiden name first. I would need to look at it to tell you the answer in the biographic section. I will later and let you know.

B and J K-1 story

  • April 2004 met online
  • July 16, 2006 Met in person on her birthday in United Arab Emirates
  • August 4, 2006 sent certified mail I-129F packet Neb SC
  • August 9, 2006 NOA1
  • August 21, 2006 received NOA1 in mail
  • October 4, 5, 7, 13 & 17 2006 Touches! 50 day address change... Yes Judith is beautiful, quit staring at her passport photo and approve us!!! Shaming works! LOL
  • October 13, 2006 NOA2! November 2, 2006 NOA2? Huh? NVC already processed and sent us on to Abu Dhabi Consulate!
  • February 12, 2007 Abu Dhabi Interview SUCCESS!!! February 14 Visa in hand!
  • March 6, 2007 she is here!
  • MARCH 14, 2007 WE ARE MARRIED!!!
  • May 5, 2007 Sent AOS/EAD packet
  • May 11, 2007 NOA1 AOS/EAD
  • June 7, 2007 Biometrics appointment
  • June 8, 2007 first post biometrics touch, June 11, next touch...
  • August 1, 2007 AOS Interview! APPROVED!! EAD APPROVED TOO...
  • August 6, 2007 EAD card and Welcome Letter received!
  • August 13, 2007 GREEN CARD received!!! 375 days since mailing the I-129F!

    Remove Conditions:

  • May 1, 2009 first day to file
  • May 9, 2009 mailed I-751 to USCIS CS

Mhay just renewed her passport at the DFA & it doesn't have her married name. I submitted the papers with the way she wanted her name shown. The visa will be issued in the name in the passport. It is better to renew now than when you are in the US. It is cheaper too.

16 March 2012 Sent I-751 package from Aviano AB, Italy.

29 March 2012 Received everything back...wrong fee. thought we didn't have to pay biometrics since we were sending fingerprint cards and passport photos.

30 March 2012 Sent everything out again from Aviano AB, Italy.

10 April 2012 Check cashed

17 April 2012 Received NOA1 dated 6 April.

06 Dec 2012 Received 10 yr green card. Letter said it was approved 28 November 2012.

My bad...I also meant to say maiden name above...

Hopefully the OP has the machine readable one...I know if you dont, you need to apply at the nearest consulate in person.

As far as dropping your maiden name completely (this is different from the name norm for filipinas), that is what you need to tell the consulate when you renew your passport.

thanks for all of the replies. I do have a machine readable one... I printed out the amendment forms and the report of marriage form along with the affidavit already so I guess from reading some of these posts, all I would have to do is submit them, and then REQUEST the consulate to completely drop my maiden name..right?
