DS-156K form

hi guys, as i'm searching for my DS-156k form, i remembered when i had my medical at st. luke's they gave us the form.. and i fill up that part cos i thought i have to :( and put ST. Luke's Hospital. did i make a mistake? if so, why did they not tell us if we made mistake when we pass all the forms? what should i do now :( i'm messing up already :crying: i know i did stupid thing not to read it carefully, but i was rushing to fill the forms :(

and by the way, i don't have DS-157 :( what is that? i fill up a supplemental something for my lawyer last time but he didn't gave me a copy..

For the DS-157 use this link DS-157

For the DS-156K use this: DS-156K

Scroll to the bottom and select the DS-156K link for the on-line form you fill in and then print out so the all important barcode is added.

For the DS-156K use this: DS-156K

Scroll to the bottom and select the DS-156K link for the on-line form you fill in and then print out so the all important barcode is added.

thanks for the links.. is the DS-156k form filled up at St. Lukes is separate from the one i should bring to the embassy? so should i bring a copy of DS-156k for my interview even i fill up before for my medical?? or they will forward it to the embassy w/ my passport and medical results??

I could be wrong, but I the DS-156K is filled up and printed (2 copies) for you to take to your US Embassy interview (along with the DS-156, also 2 copies...and the DS-157, also 2 copies)...so I have no idea why you have it at St. Luke's. Are you sure it was the DS-156K?

AOS/EAD Timeline (More complete information found here)
09-09-2010: Sent I-485 and I-765 (renewal for me, and first application for Pippo) via USPS with Delivery Confirmation.
09-20-2010: Received NOA1 hard copies in the mail (Receipt Date: 09-17-2010)! :D (09-22-2010: Check was cashed.)
09-27-2010: Received biometrics appointment letter; appointment at Jacksonville ASC on October 14, 2010 8:00 a.m. :D
10-12-2010: Mailed off our ASC Biometrics Letter to Jacksonville, FL to reschedule our appointment :( Sent it by USPS Express Mail
10-23-2010: Received mail from Jacksonville ASC dated October 15, 2010, biometrics appointment successfully rescheduled to November 12, 2010 at 10:00 a.m.! :D
11-12-2010: Biometrics complete, all files touched! Found out Pippo's actual A# It was almost not funny!
12-11-2010: Received AOS Interview Letter dated 12-03-2010. Interview on January 12, 2010 at 9:00 a.m.; Pippo's interview same day at 9:30 a.m.
01-25-2011: Kaye and Pippo received 10-year green cards dated January 20, 2011 :D Woohoo!
