DS-156K form

hi guys, as i'm searching for my DS-156k form, i remembered when i had my medical at st. luke's they gave us the form.. and i fill up that part cos i thought i have to and put ST. Luke's Hospital. did i make a mistake? if so, why did they not tell us if we made mistake when we pass all the forms? what should i do now i'm messing up already i know i did stupid thing not to read it carefully, but i was rushing to fill the forms

and by the way, i don't have DS-157 what is that? i fill up a supplemental something for my lawyer last time but he didn't gave me a copy..

For the DS-157 use this link DS-157

For the DS-156K use this: DS-156K

Scroll to the bottom and select the DS-156K link for the on-line form you fill in and then print out so the all important barcode is added.

thanks for the links.. is the DS-156k form filled up at St. Lukes is separate from the one i should bring to the embassy? so should i bring a copy of DS-156k for my interview even i fill up before for my medical?? or they will forward it to the embassy w/ my passport and medical results??

I could be wrong, but I the DS-156K is filled up and printed (2 copies) for you to take to your US Embassy interview (along with the DS-156, also 2 copies...and the DS-157, also 2 copies)...so I have no idea why you have it at St. Luke's. Are you sure it was the DS-156K?
