Civil Surgeons in TX (Houston area, Stafford or Rosenberg specifically)

Hi,

As above, really. I'm trying to find a civil surgeon in or near Rosenberg or Stafford. USCIS gave me the following:

Dr. Wojciech S Witkowski, Stafford Occupational Health Center and Urgent Care

Has anyone used this one before? On the phone the doctor was very uncertain that I wouldn't need vaccinations/a physical, which doesn't give me great confidence that he won't try to give me tests and such, despite my copy of the DS-3025 worksheet from London. He did say it'd only cost $30 if it's only paperwork I need, though.


Anyone got any experience with these, too?

I haven't used any of them, but I will be needing someone to use in the near future.
I'd kept these bookmarked from a topic a while ago, they're not stafford or rosenberg but are houston -

Also another person Dr. Forough Farzani, Hillcroft Physicians - 6400 Hillcroft, Houston, TX 77081 (713) 988-3921, They were also quoted $25 for it and I think they're a little closer to Stafford

Thanks, but that first person you suggested is the same as the first I noted, who charges $85 for the full medical (ONLY - you can't get the record transcribed without going through the medical). It's likely that the same will be true whichever office you use.

Those are all mighty fine prices that you've been quoted -- many civil surgeons charge $200 or more, or want outrageous fees simply to sign the I-693. Pick one and go -- and, if it turns out that you'll be ripped off, leave and schedule with another.

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Oh sorry I must've not been paying enough attention! I hadn't been quoted anything just copied and pasted, I guess it must've gone up in the few months since they got theirs done! Good luck in finding someone.

Well, after a disastrous attempt to get my I-693 signed with the first surgeon I suggested (Dr. Wojciech S Witkowski), I eventually went to:

Much, much, much better! Even the receptionist on the phone knew what I needed, and understood my request. And she only charged $25! Definitely recommended.

A tip for anyone who's looking for a civil surgeon: if they don't understand your request (to merely get your vaccinations transcribed to the I-693) on the phone, then it's unlikely they'll understand in person, and they'll almost certainly try to get you to do the physical and/or vaccinations again.

Dr. Tan was out for the week when I needed my vaccinations transcribed. I went to Dr. Witkowski instead and at first they weren't sure why I just needed the vaccination part. The print out of the I-693 instruction sheet proved helpful. In the instruction sheet (uscis.gov -> Forms -> I-693 -> instruction pdf file), it states that if you have completed the medical with incomplete vaccinations (K1), you just need the vaccinations part of the form filled out. The nurse photocopied that part of the instructions for their record then they agreed to do just the transcription.

Got my flu vaccine for $30 and paid $40 (they say $30-$40 but I didn't bother haggling to $30) for the transcription. The doctor wasn't in that day so I had to come back in the afternoon to pick it up. Wait wasn't that long.

My husband and I are a little closer into town, but we went to this clinic and saw the doctor there, and she was extremely reasonable and understood what we needed:

I spent a lot of time calling around to find a doctor, and most of them had no idea what I was talking about. The nurse here knew right away, and I can't remember what they charged but it was in the neighborhood of $40-60. I had my husband (then fiance) get all of his vaccinations and records together before he came to the US because I knew that they would be way cheaper in Mexico than here. Another thing I liked about this doctor was that one of my husband's vaccination records was unclear, and she gave him the benefit of the doubt and completed the form for us instead of charging us an arm and a leg to have him re-vaccinated. This is not a fancy clinic or anything like that, but I would recommend it.
