If i'm not mistaken, Ho khau is the family regristration book and phieu ly lich tu phap is the police records. there are no english forms for those. for the ho khau, you just take it to a translation place and they'll translate it for you and put their stamp on it. I think ours just looked basically like a table you would create in Word. As far is the phieu ly lich tu phap, you would get that from the local police filled out and then get it translated. Hope that helps.

Hi What's next,

Is that possible for you to pm me the screen shots of yours, both form ? I do have "Ho khau" + "Phieu tu phap ly lich" but it is in Vietnamese. I liked to translate them in English (NVC requirement) . Or where did you have it translate from Vietnamese to English ? I am in Minnesota. Is there a place in HochiMinh provide this service ?

Thanks,

Trong

Sorry, I haven't kept those documents when I came to U.S.A. But, in Ho Chi Minh City you can go to "Phòng Tư Pháp Quận" to translate and get the "Notary Public" of those translations.

Sorry dude, I think we left them back in VN. There should be a whole bunch of places that does translations in HCMC. I would go out to the consulate, talk to one of those guys on the bikes across the street. they should be able to lead you in the right direction.

Yes, just get them translated there in VN. It's not a US form, it's a VN (or fiance's home country form) translated into English.
