I am currently in the United States under a valid K3 Visa Status. The place of my port of entry to the United States last May 03, 2008 was in Detroit, Michigan. My husband, _________, and I, _________, are filing an I-485 APPLICATION TO REGISTER PERMANENT RESIDENCE concurrently filing I-765 APPLICATION FOR EMPLOYMENT AUTHORIZATION.

Enclosed please find form I-485, Application to Adjust Status together with its supporting documents, and form I-765 with its own separate copies of supporting documents.

Form I-485 Packet and Supporting Documents:

(Application for Adjustment of Status)

1. Payment in the amount of $1010.00 personal check made payable to U.S. Department of Homeland Security.

2. Form I-485, signed and dated by _________.

3. Photocopy of Notice of Approval for I-130 Petition.

4. Medical Vaccination Form 3025 and Supplement Form I-693 sealed envelope from Civil Surgeon.

5. G-325A form, Biographic Information Sheet (4 pages).

6. 2 Passport Size/Visa Photos with name and Alien Receipt # printed at the back.

7. Copy of Birth Certificate.

8. Copy of Passport Biographical Page and K3 Visa Page.

9. Copy of Form I-94 (front and back)

10. Copy of Marriage Certificate

11. Copy of Divorce Decrees for the Petitioner

12. Notarized Form I-864, Affidavit of Support – signed and dated by Petitioner

12.1 Certificate of employment

12.2 6 most recent pay stubs

12.3 Tax transcripts from 2005, 2006, 2007

12.4 W2’s from 2007

12.5 1040 IRS Form filed Year 2007

12.6 Most recent bank statement

12.7 Biographic page of Passport

12.8 Copy of Birth Certificate

1. Copy of Passport Biographical Page and K3 Visa Page

2. Copy of Form I-194 (front and back)

3. Copy of Marriage Certificate

4. 2 Passport/Visa size photos with name and Alien Receipt # printed at the back.

Copies of documents submitted are exact photocopies of unaltered documents and I understand that I may be requested to submit original documents to an Immigration or Consular officer if requested.

I didn't loook at your list but I did notice something else:

Remember, AOS is something YOU are sending in. Your husband/wife isn't sending in a petition like the K-1 petition. I don't think you need to have 2 signatures.

And I don't believe Petitioner and Beneficiary are correct terms to use anymore.

A short two-paragraph cover letter will suffice. No one at USCIS has time to read all this....they simply go straight to the required forms and other documents and digitize them.

like it's a preference not a requirement..

well.. i think il just make one since it looks formal that way.. ;)

hi!.. i was looking thru the site and i saw ur posts. i entered the US with K3 visa and K4 visa for my daughter. i would want to know if you also have a K4 beneficiary?Coz Im having a hard time regarding my daughter's papers. i don't know where to file her documents? i already have an approved I 130 petition. do i have to file a I130 petition for my daughter also? coz my husband didn't file her a petition for I130 before. hope you'll help me with this. thanks!
