Hi Guys
With regards to the packet 3 from London embassy, am i right in saying that all i need to send back to them is the first 4 sides with the checklist completed and the signature part filled in?
Also, does it matter if some of the things on the checklist arent ticked (such as divorce certificate) as these dont apply to me...should i just tick them anyway or leave them blank and write a covering letter or something like that?

I send in all the pages they mailed me ,( not the I 134 since that wasnt ready , brought it with me to the interview ) also dont forget to download the DS 156 and Ds 156 k ( I made 3 copies 2 for them 1 for my records ) what didnt apply to me on the checklist I left blank , had no problem at the interview

Do i sign DS156k or leave it blank???
It says "Do not write below this line - the consular officer will assist you in answering this part"
