Form I-765, I 485 and I 131

When it come for me to fill them out I have a couple questions..

1 Do I use my married last name? and sign with it as well?

they also ask me for my I94 # where is this located?
In my passport I have the Departure Record part? and I supposed ot have something else? was I suppose to pick up the arrival part?

Do I have a non-immigrant visa number? where is it?


is there anything else I should stumble into that I may need to know?

Use the name that you want to be on the green-card, we used married name.

I-94 # is the printed number on I-94 (Departure Record) card stapled in passport.

Non-Immigrant visa number is the red number on your Visa.
