I can only tell you that I had to have a police certificate and that came from Bonn so that might be the correct address.
Do you know what other documents I need to include for the request? Just want to do this right. Thank you
Your fiance needs to go to German embassy in Makati to get the form. Tell her to bring the original and a photocopy of her passport . She also need to bring a valid I.D to be able to get a visitor's I.D to get in. After getting the form from the pre-screener she will have to proceed to Window 1 to submit the completed form and the payment (I think it was 2,500. pesos I'm not sure)) After the consul signs the form she will be instructed to go to the bank where she has an account to do the money transfer, its fee is 15.00 Euro. Have the form and the receipt photocopied and send it to Germany.

The address of the Federal Justice Ministry is indicated in the form. It will take 3-4 weeks to get it. When she receives the clearance she needs to have it translated and notarized. I had mine translated in this office:
