Full Version: DS 230 - Documents to Send in?

When filing out the DS 230, you do have to complete both Parts I and II, correct?? It's just that, at time of interview is when the applicant signd part II right??

Also, my understanding is the following documents need to be submitted to NVC with the DS-230 - Please correct me if this is wrong or if I am missing any documents:

1) Original (or certified copy from issuing agency) Birth Certiificate with English Translation
2) Original (or certified copy from issuing agency) Marriage Certiificate with English Translation
3) Police Record (also English Translation).
4) Copies of Passport pages showing biographical data.
5) Passport photos of the Applicant (??) if so how many??

Also, does anybody know what is criteria for sumitting english translations of documents?? Do translations have to be signed AND 'Notarized' or is translation only sufficient (signed of course)??

Please advise. Your help is greatly appreciated.

Yes. Parts I & II need to be filled out with the beneficiary's information. But only Part I is signed.
