hi all

need ur help in filling DS-230 I

1. Point 1 we hv to write family name, first name, middle name...............now i hv never used my family name( in passport educational degrees etc)..........now should i write there N/A or leave it blank????

2. with DS-230 form a photostat copy of passport's biofraphic info is sent........... now my problem is that ...there is little amendment in address( amendement is that where i live now....this previously fall under sme other district , and now district is different) .........now should i get my passport amended????


pls help i m really confused.

1) What does birth Cert say for family name? N/A is not an acceptable answer it implies that the question does not apply, "NONE" is a better answer. On lines for other names used you should indicate family name as per what is on your birth cert.

2) To be on safe side get passport amended to show current address.

even in birth certificate no family name is mentioned......now what?

Then put "NONE" there, you will be fine! My Wife had no Middle name, we put "None" there.

Again N/A is only used in case where something truly does not apply, in your case you could have a family name but you don't, so "NONE" is the correct answer.

I think family name is your last name. Isn it? The reason I said that is because you said they ask for family name, first name, and middle name but they didn't ask for a last name. right?

ofcourse i know they asking for last name......i know that

but my que is ...i hv never used lastname/family name on my passport and educational degrees..........now what sud i write there??? since,if i write there my last name, i'll no proof to show this.........
