USCIS screwed up the paperwork the first time. Their computer went belly up the second time and they finally got it correct the third time.

Wow! But then again it is the USCIS 
I had to do mine again for AOS because they didn't get good enough prints the first time around. They sent me a letter saying that the biometrics weren't done right and to go back to the center at my soonest convenience for a repeat. I don't think they gave me a specific appointment. When I got there I just gave them the letter and they fixed me right up - if the first technician hadn't been a total moron I wouldn't have had to trek back there again!
