Alright, so we got the 2nd year contract and are ready to send it off probably by Wednesday of next week, so HOW do we assemble this package??? do we use those "acco fasteners" like we did in the original process??? and do the holes have to be punched at the top??? do we use a manilla folder??? What do you write in the cover letter????

I put all my evidence into sections... Financial, Banking, insurance, Housing, Vacations and other evidence. under each heading on my cover letter I listed each item, each section was separated by a divider and hole punched at the top. Photographs had a discription on the reverse and were in date order inside a small ziplock bag that was also hole punched at the top. The cover letter was on the very front with the check attached to it with a acco clip and the whole packet was inside a manila folder with tabs at the bottom and my name and A# on the front cover...

We used paper clips. A cover letter describing the attached content and I've numbered each page.
