Im a little confused with the ead form number 16
Go to Part 2 of the Instructions, Eligibility Categories. In the space below, place
the letter and number of the category you selected from the instructions (For
example, (a)(8), ©(17)(iii), etc.).

do i just put (a)(6)? and do i mail it to the chicago address ( with my I-485 forms) or the local ins address where i live? The instructions says to mail it to the local ins address. I just want to make sure on this. it is also asking for 2 passport pictures. Is that different from the i-485 passport pictures that i will send?

NO - apply as C9 if you're applying for the EAD at the same time as the GC.
I applied for my hubby as A6 and had I not caught that mistake before a decision was made I'm sure he would have been denied since that is the code for fiances.
