Hi everyone. I started to panick thinking about this but before I completely lose my mind I wanted to get the advice of VJers first.

Ok. My wife is from the Philippines (so am I but I am a USC) and upon marriage, a female's maiden name becomes her middle name (i.e., if her maiden name was A B C, her full name after marriage would be A C X with X being her husband's name). Knowing what the middle name is here in the US (equivalent to a second name), I have filled out all the forms with her married name and leaving the "Middle name" questions blank but putting in her maiden name in the "other names used" field when asked. We've just sent in the payment for the IV bill and we are now expecting to receive the DS230 packet. She had recently applied for a new passport (while all the paperwork for the CR1 was being processed) and with it showing her maiden name as her middle name ( as per the Philippines' customary way of treating the middle name).

By now, you probably know what my question is but just in case--So should she fill her DS230 out any differently from how we have filled ot all previous forms (not putting a middle name and just putting her maiden name in the "other names used" field) or should she now put in a middle name since that what it has on her passport?

I'd like to think that consistency should prevail but maybe that's just me over rationalizing things. I just hate to have to start from scratch and have to redo all the forms. We've waited long enough. This was totally neglecting the importance of attention to detail.

Please let me know what you all think.

I'd go with the way her name reads on the passport. Maiden name as middle name.
Especially on the DS 230 as what you put there will be on her green card.
Attach a letter of explantion when you submit the forms to NVC.

She is totally fine with not having her maiden name appear on her greencard if it means that there wont be any issues with the consistency of the paperwork.
