Hi everybody.
after submitting packet 3 form DS-230 the NVC said I missed some information and after talking to an operator he told me the missing info is a copy of my passport expiration date page.
anyway now I don't wanna wait for the letter to come from the NVC so I'm wondering if somebody has or knows how to make the coversheet that NVC sends that's called Checklist Letter Response that I have to send back with the missing info.
I looked at James shortcuts but they don't have it there.

The only checklist I can think of is the actual DS-230 but who knows if it will be different from the one they said they just sent you. 
Here's a copy of the checklist that goes with the DS-230:
When we submitted our DS-230 and we haven't heard from NVC, my husband called them to follow-up on our case. They told him we have missing documents. But I know I submitted all the necessary documents in their checklists. Anyways, they told my husband that we need not have to wait for the letter and that we can submit the missing documents to them so it can be processed.
This is what I did: I made a cover letter addressed to NVC stating what happened.
ATTENTION: CMR
NATIONAL VISA CENTER
32 Rochester Avenue
Portsmouth, NH 03801-2909
RE: _________ (write here your case number)
Petitioner: ____________
Beneficiary: ___________
Then write your letter.....
Then write your case number on the upper right hand corner of your documents (each page, if there are more than one). So if it gets lost/misplaced, they can easily find which file to put it in since it have a case number written on it.

Thank you guys but that's not what I need.the coversheet I need is the one they send when information is missing,its not like the one they send with form DS-230.
