My wife never used her last name until her passport was made so her BC had only first and last name. I called NVC they said you have provide baptism or affidavit by parents mother or father or uncle

or aunt stating that they were present at time of birth (Parents mother speically is good because their is no doubt that mother wont be present at time of birth ha ha ha just kidding biggrin.gif ) So we did that part. I

am wondering If anyone else had to make such affidavit is their anything else we should bring into consideration. We just trying to get everything organized before we get Packet 4

Hi, we did K1 visa and my now-husband could not obtain a birth certificate; he got the affidavit from his mother, it was on Rs 10/-- stamp paper and signed by an advocate/notary. It was accepted fine at the VFS office and Consulate in Chennai. yes.gif

Just in case, you can also provide a caste certificate and 10th class marksheet which shows information of birth date and name of father on it. We had both of those and provided them too.

yes thats a good idea !
u might also get it sweared from 1st class magistrate but the procedure is bit lenghty but considered more reliable , at times when the sirnames are common !
