I am using option 1) first middle last.

I noticed that the DS3032 / AoS package that I received via snail mail showed the name in full whenever they mentioned my name or my wife's. I am assuming it will be the same on upcoming forms. I'd rather be explicit and type out the full name to be sure.

Exactly, I assumed the same thing and typed it just the way the DS-3032 showed it, which is the agent's first, middle, last.
