I have the check list letter and the medical appointment info with the questionnaire, so it looks like all is good! Thanks for the reassurance.
I assume for the check list, for Date of Travel and Date of Wedding, it's ok to put "Currently unknown" since they say many times not to make plans until you have the visa in hand!
You can write whatever you want there - what you said, or "ASAP" or a projected date (as in, just write in your best guess). But anecdotal evidence suggests that if you put "unknown" or "ASAP," the Embassy won't mind letting you wait a bit as they schedule the people who put in hard dates. If you really don't care when you go or you know you have a lot of winding up to do (selling a house, etc etc) then put "unknown." But they wouldn't have those lines on the form if people were "supposed" to leave them free of actual information.
If you have a travel and wedding date in mind, write them in.
