Hi. I just received the interview packet and I noticed that there is a "Statement of Marriageable Age Application" form. Does the petitioner or the beneficiary need to complete this form? Or, do both the petitioner and the beneficiary need to complete this form? Thanks for your help.

I believe it's only for the Brazilian. I'm not sure because I (the USC) went to the interview.
It doesn't say in the instructions?

You folks got me a little worried. I thought I had everything filled out for her. On the Brazil embassy website.

I did not see anything about a "statement of marriagable age application. Maybe there is and I didn't see it. Is there a DS# or other document number assigned to that form? Is it downloadable? What packet does it come in? When does the SO have to turn it in? I got a list here of the stuff you are supposed to turn in at the day of the interview and I did not see this mentioned.

Citibank fee ( U.S. $100 )

Sworn Statement

Birth Certificate and copy

Police Certificate from “Segurnca Publica” and a copy

Police Certificate from “Policia Federal” and a copy

Medical Exam (sealed the way it was given by the doctor)

Affidavit of Support

Form I-134

Last Federal Income Tax

Proof of Citizenship or Greencard

This is for the brazilian to sign, AT the interview. Do not sign beforehand.

In fact, do not sign any forms that you have to take to the interview with you. They are to be signed in front of the interviewer.

They come with packet 4. You can also download from the USCIS website or the Rio Consulate website (that link you have above). The ''statement of marriagable age application'' is the last page of the form FS-257a they'll be sending you.

It's no big deal. My husband never received packet 4 in the mail and everything went smoothly at the interview.
