How do I get an I-693A to submit with my AOS application?

Visit your local Civil Surgeon, and have your vaccinations transfered to an vaccination supplement (I-693A) the civil surgeons have these forms.

Don't forget to bring a copy of I-485 instructions with you so that you can point out in the instructions that K- visas only need the suplement not an entire medical done.

to expand a bit on the above - the da-3025 form you received in the medical overseas is transcribed onto the i-693a. you do not need another medical if done within a year of the medical overseas as pointed out above. the cost i've seen for the transcription ranges between $20-60. the i-693 should be given to you in a sealed envelope for aos submission and you should get a duplicate copy for the duplicate aos package you make just in case uscis loses something.
