Hi everyone,

Do you know where can i get the latest I-134 form? the one posted in this website has an expiration date of 04-30-07.

I have 2 children, and they are listed in the petition (which is already approved by the way). Do we need to submit 1 affidavit of support only for the 3 for us and include all our names in question # 10 of the I-134 form or is it 1 form for each of us?

How about on question #11, is the answer supposed to be N/A?

Vilma,

That is the latest I-134 form. If that is on the USCIS site now, that is the latest. To keep your mind at ease, the govt agencies are accepting even 3 older versions of the I-134.

"Versions 12/04/06, 7/14/06 and 6/17/06 are also acceptable"
