It says "Arrival and Departure information" because it's a two-part form with one part for arrival information and one part for departure information. On arrival fill out the arrival information. The immigration officer will keep the arrival section and staple the departure section into the passport on the page opposite the visa. On departure fill out the departure information and the airline staff will remove it when checking in for the flight. Unless the USCIS has taken it already.
