Hi all, my SO went in to get the police cert. The US embassy website says "The Subject Access Form that you are required to complete is the Police National Computer Form (Form 3019B). " He asked for it, and they gave him form 303 (and called 2 other stations and they said it was the right form). Anyone know if this is the right form/why the #'s are different? Thanks!

Does it have a blue band across the top of it? Doesn't really "look" official? Then that's "The One".
