I'm in the USA, and wife in Philippines wants to move to another apartment.
I don't know who to notify or what forms to use to inform of her new address.

As long you are the Agent of choice, it should be ok. Then you need to call NVC to update them of her new address or ask them if you need to sign a form for change of address.
