Hello,

My wife doesn't have her name on the Birth Certificate so I contacted the Consulate which stated the following:

"Instructions for Affidavit of Birth:
a notarized affidavit from a close relative (a parent, a close relative, a neighbor or friend who was present at the time of your birth) on the appropriate stamp paper which must be sworn before a First Class Magistrate. The person making this affidavit must state how he/she knows your family and how he/she knows the facts of your birth. "

Now I have composed a letter and would really appreciate if some one could either give some modifications to the text or addition that I should add to it, following is the affidavit:
