Thank you everyone for the well-wishes! I am really excited!!! I do have a question though. I had my TD shots done today but instead of getting it from a Civil Surgeon and pay for it, my fiance and I went to City Health Department (for free). Now my question is, where can I have all my immunization records transferred to form I-693A (Supplemental)? I asked the nurse who gave me the shots and she said they can only transfer the TD shots I had but not the rest of the immunization I had at St. Lukes during the medical. Is that right? Where can I get this I-693A form? I searched for it at USCIS website and didn't find it but the Form I-693. Do I still have to go to a civil surgeon just for that form even though I already got the shots I need?

Thanks for the input and hope everything is well with you all!

Mae...

Yes! I have only recommended going to a civil surgeon that is approved by the USCIS in order to have the DS-3025 (vaccination supplemental given to you by SLEC) along with your record of the TD shot transferred to the I-693A "Supplemental Immunization Record". All of the civil surgeons already have a copy of the I-693A in house and the beneficiary does not have provide a copy of the form. You can find the link to the civil surgeon locator in my wife's signature (a link to her profile is within my signature block). You might want to also check out the AOS Forms and Tips link within my wife's signature (review Tip #2).

Cheers!!!

Link for the USCIS Civil Surgeon Locator:
https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV

Link for I-693a form:
http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf

It is preferable that Civil Surgeons (accredited by USCIS) transfers vaccination records including the TD shots done ath the City Health Department to the supplement vaccination form.

Fely and I have finally made it to California. Our plane landed late last night (Monday). Immigration was fairly easy and quick (maybe 10 minutes after initial fingerprinting). The officer had us scared briefly, because she wanted to see our medical results, which I did not have. After looking through the papers from the brown envelope several times, she finally found the med results. She also asked about our wedding day, and I said "Sept 1", but the papers from the envelope said Sept 11. They had included the marriage certificate from the I-130 submission, and not the NVC submission. So I had to explain why the two different dates, which she accepted. Then we passed quickly through customs, but missed our bus by 10 minutes. So we had to wait 2 hours for the next one.

We're at my mom's now for a few days, and of course trying to adjust to the time difference and all those other differences many of you have already experienced. Fely's is very quiet (as always), but I hope she'll become used to hearing and speaking English soon. Right now she's busy chikka texting with friend's and relatives back home.

Life is good, being together is better than anything. Thanks for all the support and I'll stop by here now and then when I have time.

BTW PW, our visa delivery was July 2, 4 days.

I had my interview yesterday, it went fine... nothing out of the usual questions posted here. I was assigned to a lady consul, she's very direct and brief, asking questions one after another while she browse on the documents i submitted to the filipino immigration officer and returning them to me one by one. She did ask me on the latter part what i do here for a living and when i told her i used to work as a system analyst, she took a look again on my form and made a comment on the university which i went to in college. After that she said ok and told me to take a seat outside and wait to be called again. I was then called to another where i was told to proceed to delbros counter and pay for the delivery fee, they gave me back the appointment letter together with the pink slip. I was told to go home after paying and wait for the delivery of the visa.

I was never told if i am approved or not, but i am hopeful that i am.

congrats! its a good sign...they never tell you anyways unless if we dare to ask. lol... but i bet you'll have your visa soon kicking.gif kicking.gif kicking.gif

hi fellow vj'ers!

i had my early medical at SLEC last july 16-17, supposedly i am scheduled on july 30 but i prefer taking it earlier so that my anxiety will be lesser and all i would think about would be my upcoming interview..

i woke up early last monday but the taxi driver don't know where SLEC is located, so i arrived around 8AM..it wasn't easy and not a walk in the park at all! but the step by step procedure the guard gave me when i came in was very helpful plus the guide i got from visajourney..i finished around 5:45 in the afternoon and they asked me to return the next day at 1:30..

so the next day, i arrived at SLEC at exactly 1pm and go directly to the 2nd floor for the vaccination..i was only called for the shots at exactly 2:30! ohmy.gif well im afraid of needles but it wasn't as painless as i thought it would be..then they asked me to go at the ground floor and wait for my name to be called..at exactly 3:30 i was called by counter A and they asked for my passport and 2 copies of 2x2 photos and sign some forms..then again..i was asked to wait again..then i was called by quarter to 5 at counter D..the lady asked me some questions about the information i have provided in the forms i filled up on the first day..i think they are confirming that everything written there are correct and accurate...finally she gave my my vaccination record and my x-ray film! yehey!! according to the visajourney, this is a sign that i have passed the medical!! i'm so happy! jest.gif when i got home i immediately told my fiance about it and he was also happy!! jest.gif

the waiting is really killing me for those 2 days of medical but nevertheless i gained a lot of friends aside from bien(my vj twin) and jenneth are also there.. it was all worth it because it lessens my anxiety..and most of all to finally be with my one and only love luv.gif

all i have to prepare and wait now is my interview...

im not rea unsure.gif but i think i can answer that Q, if they will ask you for another test like; xray (find out positive) or find your blood positive on something, asking you to be back on another day or giving you another sked (4 sputum), and other reasons that may cause delay of your interview especially if you're taking your med close to the interview date... nevertheless it doesnt mean you'll get deny or something... so i think it means you didnt pass "that day" and have to retake again, and when they will forward your passport and med record to the embassy then it means SLEC is done with you and everything is up to the embassy already good.gif

Thanks for posting the pictures of the wedding finally. You two look very happy together. Congratulations!!!! I must say I wanted to wait till July 7th for our Vegas wedding also, but Mir was wanting to get it done sooner so she can get her EAD filed and go to work. So bored being at home alone. Anyways...congrats again!

Which hotel that you can recommend me and how long did you get your marriage license? Did you have to make appointments or reservations first...ohhh sooo much questions, sowee. Huhuhu i think we can't make it before July 30, "sayang" hehehe. But we'll still try.

QUOTE(PatientlyWaiting @ Jul 16 2007, 03:18 PM) *

There are many great but reasonable hotels to choose from in Vegas. Depends on how much you want to spend and your taste. You may want to go to www.expedia.com to check them out.

Regarding the marriage license, just show up at the marriage bureau, no appointments required. You can get it in less than 30 minutes at:

Marriage Bureau
201 Clark Avenue
Las Vegas, Nevada 89155-1603
(702) 671-0600
Open daily 8am-12midnight including holidays
Fee: $55
Bring passport or IDs

Another note about Vegas weddings.....If you are in a hurry to get your CERTIFIED marriage certificate so you can file AOS (like before the fees go up), you might have a problem. The marriage certificate has to be filed and recorded by the chapel first and they have up to 10 days to do that. Our chapel took only a couple of days though to file it. We did not try and pick it up in person, but in the mail it took us about 2 weeks to get it after we sent the request. I'm not sure if you can get it any faster if you go there in person, but unless the chapel files it within a few days and you are still there on your honeymoon, it might be a little tough to get it in person anyway.....assuming you don't live nearby.

hi fellow vj'ers!

i had my early medical at SLEC last july 16-17, supposedly i am scheduled on july 30 but i prefer taking it earlier so that my anxiety will be lesser and all i would think about would be my upcoming interview..

...finally she gave my my vaccination record and my x-ray film! yehey!! according to the visajourney, this is a sign that i have passed the medical!! i'm so happy! jest.gif when i got home i immediately told my fiance about it and he was also happy!! jest.gif

all i have to prepare and wait now is my interview...

Hi Helen,

You have been so very helpful to me during my medical exams and also the rest of the guys here in VJ.

I am still here in manila and wont leave till 3rd week of sept...got my visa in may but m still here..hehehe..because of work, i guess.

by the way, id just like to confirm if it is really required for me to take more shots like RPR ( ithink this is for syphillis) and tetmus? shot/test. My fiance inquired from one of the civil surgeons, accredited by USCIS, from georgia and he is nice to accomodate us withouth getting me through a full medical exam again with him...unlike other surgeons that my fiance talked to, they wanted me to take a full medical and they will give me shots (and charged us!), even if i took it already here...well, the first surgeon is okay even if i will take the shots over here (cheaper here)..however, i really want to confirm if they are really needed since i only had 1 shot of MMR and 1 shot of varicella in SLEC...well, SLEC required me 2 shots of varicella so i need one more...

ur help is very much appreciated..as always...

Hello again everyone!

Just arrived from back-to-back trips here. A few days after Vegas, we had to fly to Boulder, Colorado for business. It was not a honeymoon trip plus my son was travelling with us so no hanky-panky for us devil.gif . Dusa si hotDoug ko hehe.

I haven't figured out how to post my wedding photos here so please bear with me. I'll figure it out when I find the time. Busy lang talaga. I will also start updating my long-neglected list.

Thanks for the update Mae. I can relate to the homesickness. I thought I was ok here, did not feel sad or homesick for maybe 2 weeks then all of a sudden, while driving along a highway here in WA, I saw what looked like South Expressway along Laguna and then I just started feeling homesick and then cried. Bigay kaagad ng tissue ang aking bf este asawa na pala. Haaay, Philippines, kahit magulo at mabaho, it's still home. Have a fun wedding day. Remember to always smile for the camera!

Thank you for the update, Bernie. I see lots of those gleaming big trucks here in WA. Gee, how do you keep them so clean after those long trips? What a fun job! Criss-crossing coast-to-coast! But it must be painful for the bum-bum wink.gif

Hi Jeng,

Its my pleasure to help whenever I can. blush.gif Good to hear that your fiance already talked with a Civil Surgeon in his area. You don't really need a complete medical since the medical exam done at SLEC is good for one-year. Since you will still be here until september, then you can have your 2nd dose of Varicella vaccine while your still here (much cheaper). The 2nd dose of Varicella should be given atleast 4-8 weeks from the first dose (same thing with MMR). If you get it here, have a medical certification form the doctor that will state the day and dose given to you, or you can also inquire at SLEC if you can have it done there and if they can supplement it to your previous vaccine record (?).

TD shots (Tetanus/diphteria) - you will have to get this in the US when you get here (as recommended by SLEC). Since this is not routinely given in the Philippines (and is not available).

RPR is no longer warranted since we already had it done at SLEC. We had been tested for syphilis (RPR- is a screening for SY) and we should be negative for them to approve us or else we would have been advised to have a treatment if we tested positive. I saw in the forms that they had us carry while waiting at SLEC (for the PE) -> it said there "NEGATIVE". So don't worry about it, just tell the Civil Surgeon that it was already done and the medical result was included in your visa envelop that will be submitted at POE.

Hope this helps. Good luck! Have fun while you're still in the Philippines...

Back in Cebu now from my early medical exam at St. Luke's Manila. Instead of the original schedule of July 30, I had it on July 16 and 17. Everything went smoothly.

First day, July 16....I came in at St. Luke's at 5:30A.M. and I am number 65! I finished at 11:30A.M. Half day only. Second day, July 17, arrived at 9:00A.M. No more immunization for me hence, I have a complete immunization from Cebu and showed them my vaccination records from our family doctor. Two hours passed, they gave me my X-Ray film and my records. Hora! kicking.gif

Thank you so much fellow VJ'ers, you've been a great help so far! good.gif

Next step is the Interview but it won't be until August....now I am nervous again....hehehe blink.gif

Met fellow VJ'ers at St. Luke's...Rea and Jen......and all of us got our results and X-Ray films!.... kicking.gif

just a quick question, it is indicated in the Manila Embassy K1 Guide by sheriff uling, 4.3 paragraph 5 that completion of DS forms (except of course ds156 which is done online) via computer are recommended..therefore those DS forms sent to us by USE in packet form may not be used? helpsmilie.gif

Most all banks will give you a blank [stamped on the back] DS 157 for you to fill out by hand or typewriter.
Most banks won't except the form filled out online.
Doesn't hurt to have one tho.
Best to make a photocopy of the one you hand fill as you will have an exact copy.

hi rea !!!

it would look neat if you fill up all your ds forms via computer. in my case, i didnt use the ds forms in packet. when i paid my embassy fee in the bank i brought my completed ds-157 form and tried to convince the teller to validate it and he did. there is no harm in trying !!!

just a quick question, it is indicated in the Manila Embassy K1 Guide by sheriff uling, 4.3 paragraph 5 that completion of DS forms (except of course ds156 which is done online) via computer are recommended..therefore those DS forms sent to us by USE in packet form may not be used? helpsmilie.gif

i was using the barcode in the computer... although i brought the packet with my penmanship on it lol (for extra) but most of the forms i pass were from the comp.

thanks for your replies beckypua, sweethoney and ronmay, at least i am now confident that i can generate the DS forms from computer and no need to use the forms from packet 4..i will be filing out online DS156 and print 3 copies with barcode, so that i have 1 extra copy..
thanks sweethoney, finally im finished with my medical and i am so anxious for my upcoming interview..

Today is the big day! Few hours from now and I will be officially Mrs. David Cash, yeaaaa! kicking.gif kicking.gif kicking.gif I am so excited, happy and nervous. I hope I'll be able to walk to the altar with weak and woobly knees. I shouldn't have bought a 4" heeled wedding shoes if I had known I'll be this nervous. laughing.gif laughing.gif laughing.gif Oh well, I got my knight in shining armour waiting for me at the altar so I'll make sure I'll pass out right to his loving arms. luv.gif
God bless us all!

Hey....does anyone know about the K-2 visa's expiration. I know the child must follow-to-join within one year of the mother's K-1 being issued. If we apply and are approved for the K-2 within that one year, is her K-2 visa still valid for 6 months after issue (like the K-1) if that 6 month's goes beyond the 1 year requirement to follow-to-join, or is it one year complete to set foot on the USA POE? Our one year ends in May, but we would like to bring Mir's daughter here in August for the start of school so we don't have to worry about child care for the summer months when there is no school.

Thanks for the helpsmilie.gif again!

the K-2 visa just needs to be issued before the end of one year from the issuance of the K-1. after that, a k-2 Visa expiration is the same as a K-1. once the visa is issued, you must enter the USA within six months.
