Aloha kakou,

I was filling in the PDF version of the I-134 and discovered that the dates on questions 1, 9 and 10 are linked - ie you fill in the date on one field and that same date replicates to the other fields. Sloppy, sloppy design. How have you guys gotten around this??

delete and check carefully.

That is because you have "auto complete" enabled through adobe.
