Just mailed out the I-751 form after spending the whole weekend filling out the form with my wife and collecting supporting documents.

Had a question for other names used (including maiden name), we probably had to put down our name before marriage even though we no longer use that name correct?

You are correct.... put any (and all) previous names you used before you got married to your current husband.

one question, and if you never did due to a lawyer telling you not to worry about it because it wasnt ever a legal name change and just a name you used 10 years ago for less than a year?

I'm looking back now too and after reading your previous post, I realized I didn't fill in all the blanks - OH CRAPOLLA!
Of course, the instructions say to fill in all blanks and use "none" or "n/a" where appropriate, and I didn't (I should have said n/a to other names, my A#, & kids).
Anybody know what will happen now?
