My husband go to fill this form (the form is nameless) to notify embassy for readiness to attend IV interview. Questions are:

1. Date of proposed travel; does it mean the date when he intends to travel to USA?

2. On the second page, it says that you must provide the details of any children and after that it requires sigatures of the applicant and the spouse. As my husband doesn't have any children, do both of us still need to sign it on the second page (one signature of the applicant is already required on first page)?

Any idea around what dates IV interview could be scheduled (DCFing London)?
