We received packet 4 today! Just to let everyone know, in the packet there was specific instructions for filling out the new I-156 form. I went ahead and did mine last week and just now had to redo it. The only real changes were that they didnt want you to fill in the country or province of issuance for the passport in section 2 of the form and they didnt want any N/A's or None's in any blank fields. We receive ours today as well. My fiance says there are 3 forms....ds156, ds156k, and ds230 part 1. Are we suppose to receive ds230 part 2?

I dont think they need the DS part 2, maybe they fill that out during their interview or something. I made a, 'things you probably wont need, but just in case they ask, you have them,' folder. Inside that's the ds part 2. Make sure she, or you, do the I-156 online, theres a barcode on it after you print it. I dont know why they even included them in the packet.

In DS-230 Part 2 , I understand you do up to Part 2 through line 34 but no further.

I got the vacination today, it's crowded at that place. It took me a half day to wait for only 2 shots for vacination. I feel tired now. I think that I will go to the Medical Exam in the next 2-3 days. I got packet 4 with my appointment letter and one appointment letter of the next person who has interview time after me.

I came back to the Consulate to return it them and to inform them that they missed that one in my packet. I has seen a long row of people who queued for their interview. They came really early, it's around 1 PM but I saw more than 50 persons in line to come to the US Consulate already. It means we have to come to the Consulate at least 1 hour before the interview time.

I don't have the form DS-230 part 2 in packet 4. But I forgot to ask them. The best way is to make it ready for in case they may ask for.

Now is the time for packet 4's preparation. There is one couple with the woman who has the same interview date with me. I way see her at the Consulate in the Interview day.

Quick question on the form you guys got on the Package 4.


Form DS-156, and DS-156K, and DS-230 Part 1 and 2. I thought the DS-230 part 1 and 2 already get turn in on the package 4. Now we have to resubmit it again. Also, on the DS-156 and DS-156K, this need to be fill out on the Web right? not on the actual Form they sent. This is confusing, Also on the web, the only form need to fill out is the DS-156. So are you suppose to fill out the online DS-156 or not?

Could you post exactly what is says about the DS-156? This has me scrambling to fill out several DS-156's for her.

Hi Lurker,

You are supposed to fill out the DS-156 using their link to a form because it takes the information you fill in and generate a bar code which contains all of your information. You are supposed to prepare 2 of these forms.

The DS-156K is a fillable PDF file that can be filled in and printed out. You can do it by hand as well but it is neater if you use the PDF file. This document must be signed in front of the officer.

The DS-230 is the same way. You can also use the PDF file format to fill and print out; or you can fill it out by hand as well.

What is the deal with not filling out the country or province in section 2? This didn't seem clear to me.

Your fiance, who stayed far from HCMC, should come to HCMC at least before one week, to take the vaccination and medical exam. She will need to take a rest 2 days before the medical exam.

Hi Lurker,

For the issuing country, I selected Vietnam from the drop down menu, and put N/A in the state section. I believe that the instruction says not to leave any field blank on any form; use N/A for those fields that do not apply to your case.

Yes, she will go to HCMC Jan 30th with her mother.

Darn it. Ok...well i was going off the original poster and what they said by not putting N/A because they said the instructions said not to use N/A or None and not fill out the state or province of issuance. This is painful.

I just reread the instruction in pack 4, here is what it says:

Thank you very much!!! I am just stressing over this since she hasn't received her Packet 4 yet because she is Hanoi now. So I want to jump on this and get her every document before she gets her packet so we are not scrambling later. You helped me clarify everything. Thank you again.

I dont know about not leaving any fields blank. I know that once I was done with filling out the I-156 and I hit COMPLETE, it told me that I couldnt use /'s and it also said not to put none or N/A in the other surnames or nickname box unless there actual name was None or N/A. As far as they issuance place, it said just state the city, not the province or country. Ngan's was issued in Da Nang, province Da Nang, country Vietnam, we just put Da Nang only.

It says in the packet that the electronic form DOES NOT get sent to the consulate, so you can withdraw it if you need to, I just did for my third time...we dated the things 02/22/2007, crazy us.

How about putting NA or Not Applicable? if it does not accept /'s.

The link on their website take the info we entered and generate a barcode to be printed out along with form DS-156 and does not send that data anywhere else. So as Ridejewel said, you can redo it any time. It the encoded barcode is what they want for machine-readable purpose.

The Instruction in Packet 4 said clearly about "N/A", I re-checked. I can send to you the packet 4 if you give me the email address, it's rather big size, I cannot upload to forum.

Also, they sent the form DS-156, DS-156K, DS-230 to me, I can make photocopy to fulfill on it for draft first to check. Then, I have to make on computer to print out.

Same here buddy. I re-do my DS-156 for the fifth times. One thing did do wrong were putting the None in section 8 and 9. also one part of the address i can't put a / in the address box. they dont' allow it on the forms so i have to put a hyphen ( - ) instead. gosh.. I called them yesterday and they told me to call back in the middle of Feb for the interview date. I guess i will call them again next week to see i will get an interview date.


Quick question for who got their Package 4? What did they said on the Letter of interview? Can you go and get the Vacinnation before getting the package or they require you have to have the Interview Letter then you can get the vacination?

I am not sure about the vaccination, but you must have your interview letter to get the medical exam because they asked for hers when she came to IOM for her exam.

Thuy was able to get her immunization done w/out Packet 4. I know from ThaiLand posts that they can do medical before Packet 4. There does not seem to be a real reason to require it - except that it would help the facility know exactly what tests to run and how to format the reports. But then who knows...

I have asked Thuy to enquire at IOM this week because we wanted to get that done before new year. whistling.gif

Please help me to check with your fiance if she didn't have the breakfast to come to IOM early morning for medical check??

If I'm not wrong, they will give us the blood test, and the requirement for the blood test is no food before the test, only can drink water.

About the fee, we have to pay $75 (the instruction said so).

I came there but I have to register for the date before the medical check. So, I must wait one week later. It's lucky as I still have enough time.

I think you are right about the blood test, but let me confirm with her and I will let you know. That's correct, it's $75, not $60 as I said because I was looking at the an older Pack 4 instruction.

Many blood tests do not have a requirement of no food. Where do you find the requirement regarding eating and drinking before the testing ? Do they specify how long in hours ?

Hi Hien,

I just checked with my fiance and she did not fast before the blood test because they only tested her for AIDS and VD (Venereal Diseases). You should double check with them before going in just to make sure. However she overheard them asking the examiner if she had any food before the blood test. And in her case, she did have breakfast but no coffee before her medical exam.

I guess it is customary for one to fast before any blood test, especially no coffee. Usually I had to fast for 12 hours with no food nor water before the blood test.

Dear Andrew,

As Tony said, the blood test for AIDS may need no food. The food can make the test result become not exactly. When I come to the Pasteur Institute for blood test in the past, they usually reminded me to have no food in that morning. No one reminded us or no word in the instruction said about this matter, but we should pay attention.

You should come to Consulate to check for packet 4, and to say to them about the address issue to see how they say.

Your interview date is the 28th. They state that if you are 2 weeks out and you still have not received you packet 4, call them and they will mail another or come into the consulate to get it. I would go into the consulate since your addy is wrong.

Guess what guys?i called today and told the girl to correct my address,she said i should write a letter bout that or i can wait til Feb 12 to pick my packet 4,this is so good cause atleast i knew what to do now,so im going to pick my packet 4
