places i lived almost 6 months since the age of 16, they just require the city, province and country or i need to put the adress?
and can this form be filled ellectronically or have i to use the one included in my packet 3 wich would be hand written??

Yes you can ues the fillable PDF, it is preferable to help prevent mistakes at the consulate when they enter the data into their systems.
