Ok Charles, I have no idea what are you talking about, I understand the birth certificate part, but that "ds-3025" and the immunization thing ???? .. where can I read more about that ?

assuming that a ds-3025 was issued during the medical, it covers the immunizations the beneficiary received prior to the interview during the medical portion. later on in the states, you'll need to have the ds-3025 transcribed over to an i-693a supplement.
