Do you need one of these from each Affadavit of Support Co-Sponsor? Or just one from the fiance(e)?

I did one for both my fiance and joint sponsor. It says: "A signed copy of this document must be attached to each copy of any affidavit of support submitted on behalf of an applicant and presented on day of appointment." If you need another copy, I can upload it as Vancouver emailed me a copy.

You don't have to go to the trouble of uploading it though, he has a copy down in the states that he can just print out and fill in at will.
