You need to go to the website for the british embassy... Google it there you will find a link to passport services and you can down load the forms and it gets sent to Washington DC... takes about 2 weeks...

Note that you can't just have your passport amended with your new name. You have to pay the fee for a whole new passport! I was just in the same situation and it seemed like a bit of a rip, to me. But oh well ... I got a new one, anyway! Good luck.
