So, I have been reading all these horror stories on the AOS forum for people who are going through hell to get their vaccination record transferred to the immigration form the the AOS portion of their journey.
With my own medical coming up, can anyone please tell me exactly what I am supposed to ask Dr. Seiden for a copy of? Will they give a copy of the whole medical? Is it just the vaccination record? Is there a specific form AND (for any of you who have used Seiden) do I have to walk with extra cash for any of this.
Cash! Can you imagine it? Ha! I can't tell you the last time I have carried more than $20 on me - you gotta love Interac and those wonderful credit cards!

I found out the name of the form, it's I693A... who knows if they'll charge more, I can only guess yes

It doesn't appear to be on the USCIS site.
I know what you mean about cash, it makes me nervous to carry it! If you wouldn't mind posting back after your medical, my husband will have to have his there sometime in the not too distant future.

What you need to ask the doctor for is a copy of the vaccination worksheet, or form DS-3025. After you enter the US you can have a civil surgeon transfer it to the I693A form, or if you are willing to risk a RFE, you can just submit the DS-3025. It seems to go either way, some people have had AOS approved by submitting DS-3025 and some have got an RFE. We took the risk, I can let you know if I get an RFE.
My doctor did not charge more, but I went to the doctor in London. They are just making a copy of one of the pages they have to fill out anyway, there really isn't a need to charge for it. Also, it says on the sheet to give a copy to the applicant.

i had my medical done there and didn't know at the time to ask for any particular vaccination form and only sent in what i had (it's put away but i don't believe it was anything more than a sheet they provided) and i didn't have any problems during AOS. i was just happy that they didn't make me get a tetnus shot even though it was within 6 months of expiring.

I think i kinda missed a point about the copy of vaccination record thing....
So i had my medical last monday. I got my results and x-rays in selad enveloppe. I have my vaccinations record since I was a kid and everything was up to date, I didnt receive any shots. So is there anything I should ask for?
Because everything is written in my personnal vaccinations records and I can always show that to anybody. So I go back to have them make a copy of somthing or I can just go own without it right now ?
I didnt read thart much ahead yet.

I went to Dr. Seiden's in Toronto, and I didn't ask for a copy of the vaccination form, but when I received the envelope and xray, attached to the envelope by paperclip was a copy of the vaccination. I have kept this and thank them for doing this without me even asking.

Ah I went to Dr seiden in Mtl , but there was not such a sheet attached to the enveloppe.
They told me though that if the enveloppe was opened (which of course wouldnt happen) that i had to go back to them so that they could reseal it. So i guess I could pass there and they could open it, make a copy for me and then reseal it in a new enveloppe....
And is this sheet saying about the shots you received at the medical or is it saying if you have everything that was required?
I just read the I-485 instructions though and it mentions about the vaccination supplement, but there is no form number at all.

The worksheet and the supplement are two different forms -- many people (not all) have received a copy of the worksheet from their medicals in Canada.
I personally submitted a copy of the worksheet when I applied for AOS, and had no problems. Others have received an RFE and had to go have the supplement filled out anyways. It's a bit of a crapshoot, so many people have opted to go ahead and get the supplement and not risk losing time with an RFE.
The worksheet is form DS-3025.
The supplement is I693A, which is not available to the general public, only the civil surgeons have them (as far as I have seen).
Both are a record of your immunizations, either historical (taken from your records) or recent, confirming that the immunization requirements are complete.

Luckily I had the cute little yellow booklet that they record vaccinations in from birth for both me and my son, that was good enough for Dr. S. There was one missing but all I had to do was call my clinic back home and they faxed him proof of the vaccination so I was all set, didnt need to have any shots or any boosters.
yes you do need cash, I didnt have any, expected interac duh, and had to run up to McDonalds about a block away if that, there was an ATm in there.

Just ask Dr. Seiden for the vaccination worksheet. That's all he gave me and I didn't ask. When you get here and file for AOS you will need to get a I693A vaccination supplement but that needs to be filled out here not in Canada.

I called back today at the place where I did my medical and they told me I could ask an offcier at the interview to make me a copy before taking away all the papers. And if not that she would gladly make me a copy of her copy that she kept in her records!
