what is National Identification Number?
Should I just leave it blank? It won't let you put N/A when you click "continue"

I don't know what it is, so I put in "None"(I was using the e-form). You can always leave it blank and hand write in something.

I put N/A. I thought maybe NONE suggested that we're supposed to have one, but I didn't get it! haha
