hi, I am preparing to file the AOS, i am not sure if I have to send the whole medical exam(I-693) or just the vaccination suppliment (I-693A). Where can i get this exam? Can i just go to my local doctor to get this done? thanks

You need to get the supplemental done by a civil surgeon. Check here:

I would recommend you call around because the cost can vary by a considerable amount. Also, if you still have the xray from the embassy physical take it with you to see the CS, it can also save you a few bucks.

As jasman mentioned, the cost of this VAC-Sup is vary from doctor to doctor, some will charge you $600, or mine would charge $10. If your wife needs any additional shots on the list of required vaccinations, those can be done at your family doctor. Just make sure the family doctor gives you receipt where it shows he did the shots for her. Attach those with your VAC record from Vietnam and the civil surgeon should be able to issue a VAC-SUP for your AOS. Keep in mind, call around and check this list of available in your area. Price varies greatly!

chuckandkim, i called several civil doctors, some said i need to redo the whole exam because the exam was done in vietnam, the cost for the exam ranging from $350 to $490. Other doctors charge $130 to $170 to complete the vacinnationm (td and chickenpox). Does my fiancee need to redo the exam or only need to complete the vacinnation?

She should only need to complete the vaccinations.

Most doctors will only do the complete exam to make more $$ but by law if she had her K1 medical exam within the last 12 months, all she needs is the I-693A. Keep calling around. It took a little time but we found one, we ended having to pay $60 here in the L.A. area for Td shot and record transfered to 693A (all sealed up). Good luck.

I second and third the other's post... She does not need to have her VAC redone if her original VAC was done within the past 12 months. Some of them shots are medically prohibited to be administered twice within a certain length of time. Figure, you don't have to get two shots of the same thing within a year... So if the CS insists your wife needs to have her VAC redone, ask him to write down the reason and show you his insurance policies. Just in case!

All jokes aside, your wife only needs to complete her VAC. Call around is the key! Your family doctor would do those shots for a lot cheaper. Don't back down!!! If they want $600 for it, you might have been better off to drive to Philly and have it done for $10, with sealed envelop and everything!
