Hi mona lisa I guess you would write it in the way your name is on any legal documents. I would use mona lisa as your first name. Its kinda like the baptismal names some people have, not everyone includes it in documents. My husband only uses one middle name.
I'm sure someone will have a better answer for you.

Hi Mona Lisa,
I have the same problem as you do. I have two first names and two last names. I haven't changed my name yet (when I do I'm going to drop my two brazilian last names altogether and adopt my husband's).
What I did was (I'm going to use your name as an example):
First name: Mona
Middle Name: Lisa Camargo
Last Name: Smith
I did that because that's how I had in my B2 visa, so I thought it would make things easier.
Now your last name is really Camargo Smith. A lot of people in the US have two names (Like Michael Edwards) too. In my opinion you should put:
First Name: Mona
Middle Name: Lisa
Last Name: Camargo Smith
If you have other questions, feel free to PM me!
