Does the background check occur at the time they receive the NOA2 from CSC or after the embassy has received package 3?

Thanks,
Raffy

NVC can perform background check as well. Definitely they do it when they have the case before forwarding to the Embassy. The process can be delayed, if there is a name hit.

I previously got 2 visas (G-4 and F-1). Does having had a visa constitute a "name hit" or is it a name hit only in "criminal databases"?

I don't know having previous visas would cause a name hit. By the way, are you on CR-1 process or K-3 or K-1?
