I'm just about done with my wife's g-325a form. One question under (Family Name) should I reference her married last name or her paternal last name?

If she took your family name then that is what to put on that line. Then next line where it states all other names is where her maiden name is placed. As well as list any other family names if she was divorced.

I focused on the word "used name"..if she changed for ex her name on papers, etc., meaning if she uses it...then I would put the maiden name.
