Hello All.
Well its time to apply for my baby daughters passport and I think I have everything covered other than a few questions.

First is where do i get a certified copy of my passport? Is this simply a USA notarized copy?

The Embassy said I had to do an Affidavit of parentage and physical presence and this looks very straight forward other than i cant remember every month/date/year of where I've lived in the USA. Is this critical or can I guesstimate?

I don't think you would need month/date and year.. I think month and year would be good enough. But you do need to remember when you lived during those time periods.... it's important in terms of calculating your time in residence in the US which determines if you can transmit your Citizenship or not... If you don't remember where you lived, usually your credit report has quite a few of your old addresses...

Also, you should provide evidence other than the affidavit of your physical presence in the US. Good evidence of this is a letter from your high school/ primary school district, college transcripts, and copies of W-2's for prior tax returns.
