Ok, so I know that we have not reveived the NOA2 yet, but I am 'practicing' filling out the packet 3 forms and making sure I have all the information ready. Hey, it keeps me from checking USCIS every 6.5 seconds! Anyway, I'm glad I did because it took me FOREVER to think back in my past to correctly answer page 2 of the DS230! Anyway .. in reading all of the documents, I have some quesitons. Here's looking to you all for help.. once again.


PACKET 3 QUESTIONS:


Form DS230 – page 2
List all educational institutes attended:
• Exactly how far back? Primary school?


List dates of all previous visits to the US:
• Use VWP for type of Visa?
• What to do if we can’t remember all times?
• What if visited twice in one month?
• Form won’t let me put two dates and it says to “FROM/TO mm-yyyy” – this happen to anyone else?

This is what I propose to write:
LOCATION FROM/TO (mm-yyyy) TYPE OF VISA "A" NO. (If known)
Baltimore, MD 08/2006 VWP – no visa necessary
Cuba, NY 08/2006 VWP
Cuba, NY 07/2006 VWP
Cuba, NY 06/2006 VWP
Cuba, NY 05/2006 VWP
Lawrenceville, NJ 03/2006 VWP

These are the dates that we can recall that I have visited the US this year. Though we cannot recall exact dates from years gone by, on average I would say that from July 2004 to December 2005, David and I took turns visiting each other about once a month. Given that, on average, I would say that I visited the US on a bi-monthly basis for 2 – 4 days at a time. I spent a few weeks each summer with David on the lake in Cuba, NY as well.

Flights I have taken prior to 2006 include:
October 2004 David and I flew out of Buffalo to visit with my mother who was attending a work conference in Washington, DC
August 2004 I flew with my cousin to visit with her father in Miami, FL
March 2002 I flew to visit with friends in Ft. Lauderdale, FL
March 1998 drove to Long Island, NY to visit with family
March 1994 drove with a friend for a vacation in Miami, FL

Prior to this, I can only remember visiting the US with my immediate family. Once when I was about seven years old on a shopping trip to New York City and then again when I was nine and twelve to visit Disney World, Sea World and Busch Gardens, all in Florida.





Form I-134
• We have a co-sponsor. On page 1, question 3, it asks for my relationship to Sponsor. Are they talking about the petitioner or co-sponsor? If it’s the co-sponsor then would the answer “future father-in-law” be ok?

• Question 7 asks for a list of assets. If only using the salary to determine co-sponsorship, then do all the items asked for also need to be filled in? i.e. life insurance, real estate etc.

• Question 11 asks if the co-sponsor intends to make specific contributions. Is it ok to just say ‘no’?



Form DS156
• Question 29
It is sufficient to say:
“WHEN? Often, especially during the last 2 years”
“FOR HOW LONG? Visits from 2 days to 2 months”

OR... should I attach an the same explanation used for Form DS230

• Question 30
Because I am Canadian, then I enter on the VWP – should I state that in this section? Or simply state ‘no’?




Form DS156K
About halfway down, there is a list of documents needed. One of those things is a marriage certificate. Now, since they wanted to know the name of my last spouse (above) then do they also want marriage certificates from previous marriages? I have a photocopy of it, but the original certified copy had to be given in to the courts when we filed for divorce and was not returned.

Do I need to apply for another certified copy? (note – marriage took place in the Caribbean)

I've only mentioned college education. I didn't go as far as primary school, but I've read that some VJers have.

I guess they're talking about the primary sponsor, the petitioner. Anyone?

Write N/A (K1 visa application.

Thanks for your reply Mew. Yes, the examples provided are great, but even after looking at them, I still have questions. I will take your advice on the other points though.

No cause question 3 is .. "That this affidavit is executed on behalf of the following person:" so your fiance goes in there.. for the relationship it would be Fiance . I-134 is a very broad form ..

hi:

i can not recall all the times i have visited the US either. i asked the consulate about it and they said just write down all you can recall and don't worry about the other times. so thats what i did and also I added a note that there were more visits but i can not recall them.

sometimes my info did not fit in the form either, here is what i did: example: Question have you ever been in the US, How long?: here I just wrote down all the dates from-to. i basicly ignored their form-layout. i could do that because i filled out the forms with a pen by hand.

i don't know if my way of filling out these forms will make them mad but i think it was the best way i could do it.

I gave them the Info all the way back to my high-school.

this is just the way i did it, not an advise.

Don't be sorry! Thanks for the advice that you did give. And thanks to everyone else who is helping too! The answers are coming and I do appreciate it. Sometimes it's comforting just to get confirmation from others who have been through the horror processes.

For education I put from high school foward.

for the visits to The US.... just list all the major trips you can remember then just put a general statement like from such and such a date I made variuos trips the US....

**I was told by Vancouver consulate high school and on ..

List dates of all previous visits to the US:
• Use VWP for type of Visa?

**The FROM dates wouldn't fill in for me .. I added an extra sheet and listed all my visits and explained the form was not working properly no Visa needed for Canada/US visits ... List all the visits you can rem. They told me to list every trip to the US with both FROM/TO dates on the added sheet

This is what I propose to write:

**They told me all visits .. so you should list the trips prior to 2006 also , i did not list my trips as a young child ( Disney World ect )

**Yes .. whatever relashionship the person filling the I-134 has to you is fine

• Question 7 asks for a list of assets. If only using the salary to determine co-sponsorship, then do all the items asked for also need to be filled in? i.e. life insurance, real estate etc.

**I was told to leave no blanks .. fill in whats relivant and put NONE or N/A in the spots that are not

• Question 11 asks if the co-sponsor intends to make specific contributions. Is it ok to just say ‘no’?

**I think when it's time to fill the forms for real .. this is a Question you should ask your consulate to be sure

**I used the last visit date for " when " and the amount of days for the " how long " and then i added To/From dates for the rest of the visits ( when you print it the additional visits will be shown on the 3rd page with the barcode # )

** I asked Vancouver if i needed proof of previous marrigae + divorce because it also asks for both in packet 3 .. i was told "IF" it was not possible to obtain marriage cert. proof of divorce would be ok .. I also had to give my Marrige cert. to the courts during my divorce. I ordered a new one and have the reciept( if it doesn't come before my interview i will say it wasn't possible to get one ! lol )
