Full Version: Questions on forms for pack 3

On form DS-156 Q.14 it asks for National Identification Number - is this our UK national insurance number? or do we just put N/A?


Ok I have the following

1. DS-156 In duplicate as I hand wrote it - Do I sign this now? or at the interview?
2. DS-230 - part 1 - Do I sign this now? or at the interview?
3. DS-156K - Do I sign this now? or at the interview?
4. The checklist - I havent fixed a medical yet and i put a note to the effect that I am waiting for interview date so I can schedulae a medical near the time of the medical preferably same week or same day:)


I also have the midical form but i understand that has to go qwith me to the medical.

Is this all i need to send in pack 3 ofcourse with a cover letter smile.gif

I finally got the docs from my Dave in the email today so I am hoping to get this pack out on monday or tuesday - after all the telling him what he has to send he still forgot an upto date letter of intent(

On the NI number i put n/a
DS156 and DS 156K i did not sign
DS230 i did sign
and yes you take the medical form with you to the medical.....hope this helps....doing it that way worked for me...good luck

Paul's arrival in Detroit was SUPER EASY - I wasn't sure where you two were planning on having you come in, or how close you are to the Michigan border - but Paul had no problems in Detroit.

So glad to see you two are rolling on good.gif

it is written very confusingly!

So packet 3 is those forms & the checklist, that's it? Right????

My fiance mailed out his Packet 3 today! I'm just so nervous! I hope everything is right...I know how confusing those forms can be.
