Yesterday she went to get her medical exam from the Thai hospital listed on embassy website and they told her that they have never seen that form before. So, my wife ask me why I sent her to get medical exam when Embassy will send form to her later?
I don't understand. The form on Embassy website is the same form as VJ. How could it be the wrong form?
What can I do from here?
She will have to figure it out for herself.

We had a similar issue with the online forms from the Embassy website. Luckly for us Packet 3 arrived at her home the following day. Wait for the forms to arrive and everything will go smooth.
I relate with your anxiousness to get stuff done early, but you might be jumping the gun on the medical exam. You still have a little ways to go (according to your timeline). Hold off until you get the papers from the embassy...
