I need help with the following question for my fiance filling out the form DS-156 Non-Immigrant Visa Application

#2 Passport - Place of Issuance
City
Country - Guyana
State/Province

The passport from guyana doesn't list City or State/Province. It only has an oval stamp that reads Passport Office, Republic of Guyana. Do I write "None", "N/A" or leave it blank next to the words City and State/Province

#18 Spouse's Full Name (Even if divorced or separated. Include maiden name.)

This is confusing, does he write his ex-wifes maiden name?

#19. Spouse's DOB (dd-month-yyyy)

I guess this will depend on # 18 - if he writes his ex-wife name he would place her birthday here.

#20. Name and Address of Present Employer or School
#21. Present Occupation. (If retired, write "retired". If student, write "student".)

He stopped working 1 month ago - all his former applications and forms showed his then current employment. So do I write "None" or Formerly ABC Shipping Company for # 20 and "None" or Seaman/Stopped work 1 month ago for #21

#28. Who Will Pay For Your Trip?

Does it look bad if he states his fiancee?

#40. Application Prepared By:
I his fiancee and filling this out for him on the computer, I want to fax it too him, will it be accepted with a faxed signature rather then an original signature by me. It would then have his original sinature as the applicant?

#2 Where was the passport application sent to? Where was the passport sent from? If everything is unknown the answer should be .... In any event, the question is applicable.

#18 Whatever names ex-wives used, including their maiden name. Most ex-wives used their husbands name.

#19 Where does this question say to include ex-wives?

#20,#21 The question asks for "present" information. Sounds like your fiance is presently unemployed?

#28 It's not uncommon for the USC fiance(e) to be paying the expenses.

#40 Original signatures.

THe city that passports are issued in Guyana is Georgetown
